The Case Manager Substance Use Counselor Certified serves as the primary support and case manager for individuals with mental illness by assisting them to live as independently as possible in their chosen community, focusing on substance abuse and addictions. PACT is an Assertive Community Treatment (ACT) program operated by Telecare Corporation in Washington State. This program provides intensive, community-based behavioral health services to adults living with serious mental illness, many of whom also experience co-occurring substance use challenges. Grounded in the recovery model, PACT is designed to meet individuals where they are—both literally and clinically. Services are delivered in the community and tailored to each person’s unique goals, strengths, and stage of recovery. The multidisciplinary team offers comprehensive, wraparound support, including psychiatric care, medication management, case management, and skill-building interventions. A key focus of the program is supporting individuals with co-occurring disorders. While PACT does not provide detoxification services, the team emphasizes rehabilitation, harm reduction, and long-term recovery. Services are centered on helping individuals build stability through meaningful social supports, access to safe and sustainable housing, and connection to community resources. Through a person-centered approach, the program prioritizes individualized care planning, empowering members served to define and pursue their own recovery goals—whether that includes improving health, strengthening relationships, increasing independence, or achieving greater community integration.
