About The Position

As a member of the Assertive Community Treatment (ACT)/Integrated Dual Disorder Treatment (IDDT) team, the Case Manager Specialist is responsible for ensuring continuity of services, with a primary responsibility for assuring that the six case management functions are carried out: assessment, treatment, planning, linking, monitoring, and advocacy with potentially more challenging consumers. Services are delivered in community settings and are designed to assist individuals with serious mental illness and co-occurring conditions in achieving their recovery goals, increasing independence, improving functioning, and maintaining community tenure. The Case Manager/SUD Specialist is supervised by the Operations Manager as assigned. This position has no direct supervisory responsibilities.

Requirements

  • Ability to be credentialed by NLCMHA to provide services in this professional discipline.
  • Must have licensure with the State of Michigan as a licensed professional: Licensed Masters Social Worker, Licensed Professional Counselor or Psychologist, Limited Licensed Masters Social Worker, Limited Licensed Professional Counselor, or Masters Limited Psychologist preferred.
  • Licensed Bachelor Social Worker or Limited Licensed Bachelor Social Worker may be considered.
  • Must be certified as a MCBAP Certified Alcohol and Drug Counselor or be eligible to be placed on a Developmental Plan through MCBAP to obtain certification.

Nice To Haves

  • Master’s Degree in Social Work, Counseling or Psychology preferred.
  • Degree in Nursing considered.
  • Three (3) years of post-degree experience in developmental disabilities or mental illness areas preferred.
  • Experience as a case manager is desirable.
  • Experience working with co-occurring consumers is preferable.

Responsibilities

  • Complete annual assessment and person-centered planning for consumers who meet criteria for the most intensive non-residential service in the continuum of care.
  • Facilitate treatment plans which address consumer needs and include empowerment, natural supports, and community inclusion, as appropriate. Consumer needs may include, but are not limited to, housing, interpersonal relationships, community inclusion, legal, education/employment, healthcare, safety, entitlements, and recovery.
  • Determine appropriate level of care using Level of Care Utilization System (LOCUS) tool.
  • Ensure services are provided in accordance program guidelines and request alternative services as appropriate.
  • Provide crisis intervention by participating in 24-hour on call coverage on a rotating basis.
  • Functions as a primary SUD therapist for consumers with co-occurring disorders, once appropriate credentialling is obtained.
  • Collaborate with consumers (and staff) in assessing their needs, developing treatment goals, objectives, methodologies, timeliness, and evaluating progress toward goals.
  • Provide direct clinical care to consumers and their families through individual and family counseling, co-occurring services, problem solving interventions, psychoeducation, modeling and support for a range of activities of daily living, community activities and involvement, and skills required to maintain independent functioning within the community.
  • Maintain knowledge of local community resources and supports to advocate for and empower consumers to facilitate independent functioning with the community.
  • Actively participate in daily teams meetings, and routine team trainings and supervision sessions.
  • Facilitate appropriate transitions of care based on medical necessity and level of care guidelines.
  • Assist team in quarterly review of treatment plan goals
  • Provide appropriate transportation for service needs and/or assist the consumer in arranging transportation to achieve treatment plan goals.
  • Facilitate groups as requested to provide psychoeducation, support community inclusion, and build skills for independent functioning.
