About The Position

To guide, educate, and support individuals through positive goal-oriented interventions to strengthen and empower them towards self-sufficiency. Case manager will access client’s needs and then connect them with the necessary community resources by guiding them through the social support network systems. This position reports to the Behavioral Health Administrator and office is based on the KTI reservation in Usk, WA.

Requirements

Valid driver’s license, reliable transportation and be eligible for automobile insurance.
Must have or be able to obtain HIV Aids Training and a current CPR and First Aid Certification.
Must be able to pass a background check and sign a statement of confidentiality.

Nice To Haves

Responsibilities

Provide wrap around services for individuals referred from agencies and programs affiliated with the Kalispel Tribe of Indians.
Work collaboratively with Camas Path Behavioral Health, community service programs and agencies to ensure the coordination of treatment services.
Maintain client and program records in accordance with Tribal, State and Federal laws.
Work collaboratively with staff, colleagues, the Tribal and local court systems, and all other agencies that provide services to the Kalispel Community, as well as the community at large.
Transport clients and attend appointments, meetings, court hearings and other appointments in the community or out of state if necessary.
Provide outreach, intervention, evaluations, treatment planning, case management, aftercare services, advocacy and referrals as needed.
Provide clients with relevant and easy to comprehend information about tribal and community services.
Adhere to all applicable tribal, federal, or state privacy laws and regulations, including but not HIPAA and 42 CFR Part 2.
Supervise and oversee the Camas Path Transporter and the south Case Manager.
Attend training as necessary.
Additional duties as assigned.
