About The Position

Ascension is seeking a Case Manager Social Worker to join their team at Ascension St. Vincent's Riverside Hospital in Jacksonville, FL. This full-time, day shift position involves orchestrating the patient journey by leading interdisciplinary teams to design and execute seamless care plans from admission through discharge. The role champions patient-centered care by integrating family values and personal preferences into every stage of the clinical process. The Case Manager will drive clinical outcomes through proactive assessments and continuous monitoring, eliminate barriers to recovery by advocating for essential resources, and serve as a strategic lead on medical documentation and post-discharge planning to ensure long-term patient success and compliance.

Requirements

Bachelor's degree in Social Work required.
Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.
Certified Social Worker preferred.

Nice To Haves

3 years of experience preferred.
