Orchestrate the patient journey by leading interdisciplinary teams to design and execute seamless care plans from admission through discharge. Champion patient-centered care by integrating family values and personal preferences into every stage of the clinical process. Drive clinical outcomes through proactive assessments and continuous monitoring to ensure every intervention is effective and purposeful. Eliminate barriers to recovery by advocating for essential resources and resolving obstacles that impede high-quality care. Serve as a strategic lead on medical documentation and post-discharge planning to ensure long-term patient success and compliance.

Requirements

High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.

Nice To Haves

Clinical Counselor Preferred.
Social Worker specializing in Clinical Practice credentialed from the Florida Board of Clinical Social Work, Marriage and Family Therapy and Mental Health Counseling preferred.
