Requirements
- High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
Nice To Haves
- Clinical Counselor Preferred.
- Social Worker specializing in Clinical Practice credentialed from the Florida Board of Clinical Social Work, Marriage and Family Therapy and Mental Health Counseling preferred.
Responsibilities
- Orchestrate the patient journey by leading interdisciplinary teams to design and execute seamless care plans from admission through discharge.
- Champion patient-centered care by integrating family values and personal preferences into every stage of the clinical process.
- Drive clinical outcomes through proactive assessments and continuous monitoring to ensure every intervention is effective and purposeful.
- Eliminate barriers to recovery by advocating for essential resources and resolving obstacles that impede high-quality care.
- Serve as a strategic lead on medical documentation and post-discharge planning to ensure long-term patient success and compliance.
