Southwest Behavioral & Health Services is seeking a dedicated SMI Case Manager to join our Buckeye Outpatient Clinic. This role focuses on providing direct services in the home, community, or office, with a primary emphasis on supporting members transitioning from hospital care back into community-based settings. The Case Manager will facilitate and coordinate hospital discharge planning and follow-up services to ensure a safe and successful transition. Interventions can be individual or group-based. Responsibilities include performing crisis stabilization, connecting members with community resources, conducting assessments, completing necessary documentation, and serving as a central support throughout the transition and beyond. The role also involves case management functions such as behavior coaching, medication monitoring, and clinical care coordination with internal and external resources. Collaboration with hospital providers, community partners, members, and families is essential to coordinate services, address barriers to care, and support treatment continuity post-discharge.
