About The Position

Assists individuals 60 and older by providing information, assistance, and access to necessary services and supports to enable them to live independently. The role involves screening, assessing, developing, and implementing care plans with individuals, and providing referrals and linkages to appropriate programs and services. Ongoing follow-up, monitoring of service delivery, and re-assessments are conducted as needed. The position also includes planning and implementing community outreach activities.

Requirements

  • Bachelor’s degree required.
  • NYS Driver’s License that meets agency policy.

Responsibilities

  • Provides information and referral on services to the elderly, their families, and other service providers via telephone, in person, and through community forums.
  • Conducts thorough client intakes to determine the need for case management services.
  • Performs in-home assessments of clients and develops care plans to meet identified needs.
  • Implements and revises care plans as necessary.
  • Advocates on behalf of clients with formal and informal support systems.
  • Provides linkages and referrals to agencies and programs offering assistance to seniors.
  • Stays current with programs pertaining to the elderly.
  • Attends meetings, trainings, and in-services as required by the Erie County Department of Senior Services.
  • Utilizes computer software for assessments, reporting (including worker log), and correspondence, including email, scanning, faxing, and virtual conferencing.
  • Maintains accurate and timely client case records in accordance with established EISEP standards.
  • Participates in regular case conferencing sessions with supervision and consults with supervisor as needed on difficult cases.
  • Intervenes in crisis situations, making appropriate referrals to agencies dealing with medical, environmental, mental health, and utility emergencies.
  • Maintains a working knowledge of community resources to meet client needs.
  • Supervises students, volunteers, aides, or other individuals as assigned.
  • Maintains confidentiality and utilizes potentially sensitive information in accordance with agency and departmental policies.
