About The Position

Horizon is seeking a Case Manager to join their team. In this role, you will work within a supportive, team-based environment with a seamless onboarding experience. You will become familiar with the SBIRT public health model and work with students and families to determine wraparound services that may complement the adolescent/young adult’s treatment and support the family unit. You will maintain continuous contact with the SBIRT team, adolescents/young adults, and families, ensuring everyone is informed about a participant’s full spectrum of services. Additionally, you will ensure all patient data and information is properly recorded, entered, and stored, maintaining complete participant confidentiality at all times. You will also conduct grant-required surveys and data entry, assist the Project Coordinator with intake and referral tasks, participate in relevant trainings, and support program marketing efforts including community outreach and tabling events. Travel to schools/partners/community sites may be required.

Requirements

  • Associate's degree with 3 years of experience in the behavioral health field required.
  • High school diploma with 6 years of experience in the behavioral health field required.

  • Bachelor's degree in related field preferred.
