About The Position

The Case Manager RN facilitates the interdisciplinary plan of care, focusing on clinical appropriateness, medical necessity, level of care, and resource management. This role promotes quality outcomes, patient throughput, and supports optimal care while managing resources efficiently. The Case Manager RN identifies barriers to care and discharge and develops effective patient-centered plans.

Requirements

  • Minimum of 3 years of recent Case Manager experience in acute care or relevant units (Med/Surg, Tele, Neuro, ICU, PCU, ED).
  • Experience in home health or insurance considered if combined with 3 years acute care experience, including 1 year within the last 5 years.
  • Associate Degree or Diploma in Nursing required; BSN preferred.
  • Current Florida RN license or compact license with Florida RN obtained within 90 days.
  • Case Management, Nursing, or Utilization Review certification preferred.
  • Advanced Practice RN license acceptable if current and compliant.

Responsibilities

  • Perform comprehensive assessments of psychosocial and medical needs.
  • Develop, document, and communicate individualized case management plans.
  • Lead interdisciplinary team meetings for patient care planning.
  • Evaluate admissions for medical necessity and observation appropriateness.
  • Conduct utilization management reviews and communicate with third-party payors.
  • Serve as a liaison among physicians, patients/families, hospital staff, and external agencies.
  • Ensure compliance with regulatory requirements, quality initiatives, and organizational policies.
  • Make referrals for high-risk patients and recurring/chronic cases to appropriate programs.
  • Track and trend barriers to care and develop action plans for improvement.
  • Facilitate post-discharge needs and access to community resources.
  • Promote patient safety and cost-effective care without compromising quality.
  • Document all case management interventions, recommendations, and activities accurately.
  • Participate in performance improvement initiatives and mandatory education requirements.

Benefits

  • Salary Range: $31.94/hr (min) – $39.93/hr (mid, 10+ yrs) – $43.92/hr (max, 15+ yrs)
  • Shift Differentials: Evening +$2.50/hr, Weekend +$2.00/hr (stackable, up to $4.50/hr for evening weekend shifts)
  • Sign-On Bonus: $10,000 paid over 5 installments across a 2-year commitment
