About The Position

This hybrid position is for a Registered Nurse (RN) on the Georgia team, based out of the Douglasville, GA location. Adult Foster Care is a service where a patient lives with a caregiver who provides daily support tailored to the patient's needs. The program aims to offer necessary care while promoting patient independence in a home environment, with adaptable care options as needs change. It serves as an alternative to nursing facility care for individuals requiring that level of support, prioritizing patient dignity, self-respect, and privacy. In this role, the Registered Nurse will conduct clinical evaluations in the homes of Adult Foster Care members to assess patient needs for enrollment, service plan development, and ongoing compliance. They will also perform monthly visits for members receiving care from FreedomCare to ensure service quality and address any changes in their condition.

Requirements

  • Registered Nurse licensed by Georgia Board of Registration in Nursing
  • Minimum of 2 years clinical experience
  • 1 year of experience with elders or adults with disabilities
  • Valid driver’s license with an insured vehicle for travel within the state
  • Strong communication and interpersonal skills.
  • Problem-solving and decision-making abilities.
  • Organizational and multitasking skills.
  • Excellent communication, critical thinking, and organizational skills
  • Self-motivated
  • Able to thrive in a fast-paced start-up environment

Nice To Haves

  • Bilingual in Spanish is a plus
  • Experience with CRM (Salesforce is a plus)

Responsibilities

  • Provides support to the caregiver and member, and reviews services according to scope of Adult Foster Care.
  • Respects client and employee rights and privacy, ensures the security of protected information, practices in an ethical manner and is compliant to the agency’s compliance program and privacy policy.
  • Follows the FreedomCare’s clinical policy and procedures.
  • Assesses and regularly reassesses the client’s needs and their PCP summary form, treatment and education and communicates any changes to the member’s PCP.
  • Develops implements, evaluates, revises, and provides input to the client’s plan of care to include the physician’s plan of treatment, and problem list. Provides instruction of care to Caregiver according to the plan of care.
  • Completes all client documentation, to include written records, reports, and notes in accordance with agency policy and procedure following client visits or contacts in an accurate, timely, complete, concise and legible manner.
  • Actively coordinates the care of the client through communication and written documentation to the physician, RN and Program Director, other disciplines involved in the client’s care and community resources.
  • Interprets the philosophy and objectives of the agency to clients, families, and others.
  • Performs other job-related duties as may be required or directed by the supervisor.
