Case Manager RN - NICU - Part Time
About The Position
This position is accountable for assessing, planning, implementing, coordinating, monitoring, and evaluating options and services available to meet patients’ health needs. The Case Manager coordinates services for a specified group of patients, ensuring care is provided in an appropriate setting and that the severity of illness and intensity of service are consistent with each patient’s clinical condition. The role collaborates with healthcare team members to ensure care meets desired patient outcomes, uses resources appropriately, and is delivered within expected time frames across the continuum of care. The position practices within the scope of the Nursing Practice Act and in accordance with Valley Children’s Healthcare policies.
Requirements
- Two-year nursing college degree.
- California Registered Nurse license through the California Board of Registered Nursing.
- Minimum of two years of nursing experience.
Nice To Haves
- Four-year degree in nursing or a field that supports the case management process.
- Pediatric experience.
- Case Management or Utilization Management experience.
- Bilingual skills.
