About The Position

This role involves coordinating and managing patient care by assessing needs, developing and monitoring individualized care plans, and ensuring patients receive appropriate services. The Case Manager RN also collaborates with patients, families, healthcare providers, and other care team members to promote quality outcomes, continuity of care, and effective use of healthcare resources. Requirements

  • 4 years of experience - Must have recent and at least 3 years' experience as a unit-based IP case manager in the hospital setting.
  • Flu Vaccine Required - Medical/Religious Exemptions and Declinations Allowed.
  • Required Certifications: BLS
  • Active Virginia or Compact RN license in hand at the time of submission; no exceptions
  • EPIC Charting Proficiency Experience Required, no exceptions.
  • Recent experience in an adult acute care hospital setting with discharge planning
  • Minimum 5 years of recent acute care Case Management experience (hospital setting)
  • Less than a 6-month gap since their last acute care Case Management role
  • Experience in a Level I, II, or III acute care hospital
  • Previous patient census of approximately 15–25 patients
  • Rated 4/5 or 5/5 across all Case Management functions, including knowledge of CMS regulations
  • Acute care discharge planning
  • Utilization Review (UR) experience required
  • Ability to effectively manage a high-volume, fast-paced patient population

  • Teaching Hospital Experience Preferred.
  • Community Hospital Experience Preferred.
  • Trauma Level I Experience Preferred.
  • Trauma Level II Experience Preferred.
  • Preferred Certifications: Accredited Case Manager, Certified Case Manager
  • Previous travel experience strongly preferred. First-time travelers will only be considered if they have extensive acute care Case Management experience.
