This opportunity is a Per Diem Case Manager position at Wellington Regional Medical Center, a 235-bed acute care hospital in Wellington, Florida, owned by a subsidiary of Universal Health Services, Inc. The role involves ensuring appropriate utilization of hospital services, developing discharge plans, and recognizing stroke symptoms. The case manager will work with stroke patients and their families on discharge planning and treatment, based on the patient's needs, condition, and prognosis. Maintaining current stroke knowledge through annual training is also required.

Requirements

Must be a Florida RN
2-4 years recent acute care experience required
Experience in utilization chart review and discharge planning
Knowledge of InterQual Criteria
Strong clinical background needed
Strong interpersonal communication skills
Basic computer skills

Managed care experience preferred

Responsibilities

Ensures appropriate utilization of hospital services
Developing a discharge plan and all appropriate services in timely manner
Recognizes the signs and symptoms of stroke and calling a stroke alert overhead
Verbalizes stroke alert process
Works with the stroke patient and family for discharge planning and treatment based on needs, condition and prognosis of patient
Maintains currency in stroke knowledge by completing the annual 1 hour online training
Reports to the Director of Case Management
