About The Position

The Acute Care Registered Nurse (RN) Case Manager assesses and coordinates patient care across the continuum. The role uses accepted criteria and guidelines to manage benefits and determine the medical necessity of services. The RN Case Manager works to achieve optimal clinical and quality outcomes by managing care and resources effectively and reducing unnecessary utilization. The position reports to the Director of Case Management or designee.

Requirements

• RN license.
• Associate degree in nursing or higher.

Nice To Haves

• Two years of acute hospital-based nursing experience.
• Certification in case management (ACM or CCM).
• Experience in acute care hospital case management.
