Under the general supervision of a Mental Health Program Supervisor, this position coordinates the delivery of therapeutic treatment, community support, psychosocial rehabilitation, and related services for individuals with mental illness or co-occurring mental health and substance use disorders. The role primarily supports clients residing in specialized residential placements. In collaboration with treatment providers at the home, the position contributes to the development and implementation of person-centered treatment plans. It also ensures that clients access the internal and external treatment services, along with community-based social and economic supports, needed to achieve treatment goals and maximize independent functioning while adhering to specialized residential care parameters. This job is a full-time, benefited position working out of the James St. and Fulton St. office, with extensive travel to specialized residential placements throughout the state.
