About The Position

Ascension St. Vincent's St. Johns County Hospital is seeking a Case Manager Registered Nurse Lead to join their team in St. Johns, Florida. This is a full-time, rotating schedule position. The role involves coaching, evaluating, and developing the case management team, overseeing daily workflows, streamlining processes, and championing efficient resource utilization and discharge planning. The Lead will also serve as the primary escalation point for complex patient care tracking, act as a preceptor for new case managers, and collaborate with leadership on daily operations, including hiring, training, and departmental performance.

Requirements

Registered Nurse obtained prior to hire date or job transfer date.
High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degree required.

Nice To Haves

Case Manager credentialed from the American Case Management Association (ACMA).
Case Manager credentialed from the Commission for Case Manager Certification (CCMC).
Master's degree preferred.

Responsibilities
