About The Position

The Interfaith Works Rapid Rehousing Program provides supportive services to 60 formerly homeless individuals receiving temporary housing subsidies from Montgomery County Health and Human Services. The program is designed to assist individuals to maintain housing in scattered sites across the county with the goal of increasing income and achieving complete housing independence. The Case Manager will work with the county Housing Locator and Interfaith Works Rapid Rehousing team to identify appropriate housing options for participants, to help navigate their move in process, and to continually address barriers to housing maintenance while they are receiving the subsidy. The Case Manager will offer support to include connection to vocational services, community resources, rental payment monitoring, landlord engagement, and crisis resolution. The Case Manager is responsible for a caseload of 15 Rapid Rehousing participants with whom they must conduct a home visit at a minimum of twice a month. This is a full-time position.

Requirements

  • Bachelor's degree in a behavioral health field preferred but commensurate experience will be considered.
  • Excellent interpersonal and communication skills to effectively work as part of a team.
  • Strong cultural competence to serve clients from diverse racial, cultural, and socioeconomic backgrounds.
  • Experience working with acute and chronic behavioral health issues and knowledge of crisis intervention techniques.
  • Ability to apply best practices in working with the homeless population, including Trauma informed Care, Housing First, Harm Reduction and Motivational Interviewing.
  • Experience with crisis intervention, including assessing and responding to individuals with suicidal and homicidal ideation.
  • Ability to manage multiple responsibilities efficiently.
  • Ability to work independently and with a team, collaborating with other Interfaith Works programs and departments.
  • Knowledge of (and/or willingness to learn and explore) community resources related to homelessness.
  • Proficiency with Microsoft Windows and Office365, in addition to Outlook, Word, and Excel.

Nice To Haves

  • Fluency in Spanish a plus.

Responsibilities

  • Work with the Program Director and Rapid Rehousing team to ensure participants have the individualized support they need to maintain housing and achieve independence.
  • Build rapport with clients using strengths based and Housing First approaches.
  • Coordinate with county housing locator to support housing options for individuals.
  • Once moved in, complete home visits at least twice a month to support individuals in performing the tasks required to achieve housing stability, including on-time payment of rent and household maintenance.
  • Develop and update service plan goals with program participants, assisting them to set and prioritize short and long-term goals.
  • Assess clients ongoing needs and provide diverse client supports to regularly include referrals to vocational services, social security and HHS navigation assistance, life skills education, health maintenance, medication monitoring, transport to essential appointments, etc.
  • Update client charts and files regularly, documenting every client communication into a Homeless Management Information System (on-line HMIS database).
  • Maintain communication with Montgomery County HHS, landlords, and/or property management companies to ensure stability of participants in the community.
  • Assist participants to negotiate new leases on an annual basis.
  • Participate in staff and provider meetings to present cases, discuss appropriate interventions, and collaborate with other services.
  • Join the Montgomery County's Homeless Continuum of Care Coordinated Entry System calls and meetings, as needed by the program.
  • Participate in Interfaith Works meetings, including monthly Case Management & Vocational meetings, and other activities.
  • Meet with the Program Director for weekly supervision.
  • Regularly join the Montgomery County's Homeless Continuum of Care Coordinated Entry System meetings.
  • Other duties as assigned.
