Under general direction of the Director of Community Day Services, the Community Day Services Case Manager/QIDP provides training to staff and to individuals served in Community Day Services on a daily basis. The Case Manager/QIDP directs implementation of the Person Centered Plan through authoring Implementation Strategies and monitoring of methods employed, coordination of services and documentation of results; facilitates enrollment and disenrollment procedures. The hours are Monday through Friday, 8:00 am to 4:30 pm!
