We are seeking a Case Manager to provide fill-in coverage for care coordination services at KSRC Surgery and Inpatient Rehab facility. This position assists with psychosocial assessments, discharge planning, crisis support, community resource referrals, and communication with patients, families, physicians, and hospital staff. This role is a great opportunity for a licensed social worker or registered nurse who enjoys patient advocacy, discharge planning, and working collaboratively with a supportive surgical hospital team.

Requirements

  • Master of Social Work or RN license issued by the State of Kansas.
  • Ability to provide documentation demonstrating employment eligibility as required by the Immigration Reform and Control Act of 1986.
  • Ability to pass physical and drug screen.
  • Ability to pass background check.
  • Ability to pass Medicare sanction check.

Responsibilities

  • Provide fill-in coverage for care coordination services at KSRC Surgery and Inpatient Rehab facility.
  • Assist with psychosocial assessments.
  • Assist with discharge planning.
  • Assist with crisis support.
  • Assist with community resource referrals.
  • Facilitate communication with patients, families, physicians, and hospital staff.
  • Provide operational continuity during manager absences including staff support, patient care coordination, and regulatory compliance.
