About The Position

Duties and responsibilities are related to the provision of prevention-focused case management services for youth and young adults who are experiencing housing instability or are at risk of homelessness. Services shall emphasize housing stability, family engagement, resource coordination, Positive Youth Development, life skills development, and community-based support.

Requirements

A Bachelor's degree in a human service field.
Good physical condition, to include a negative TB & Communicable Diseases Screening. Must be in such physical and mental health so as not to negatively affect either the health of the consumer or the quality of his or her care.
Three good character references submitted during the hiring process.
Must pass all background clearances and Protective Services checks per state guidelines.
Must have a valid Michigan driver's license and acceptable driving record as determined by the agency's insurance carrier.
Must successfully pass all necessary training required by the State, any governmental agency, and the employer.
Any additional requirements which the funding source or the employer may impose in the future must be complied with.
Must demonstrate the ability to respond appropriately to the cultural, socioeconomic and linguistic differences present among the population served. Must promote a culture of gentleness and trauma-informed care.
Must demonstrate the ability to develop a mutually respectful partnership with the persons served, as well as with their families and guardians, so they are helped to gain skills and confidence to address any issues and problems they face.
Must have a conviction about the capacity of people to grow and change.
Must have the ability to work in partnership with other team members.
Must have the ability to set limits, maintain the role of the position, and intervene appropriately to meet the needs of the persons served and/or family members.
Before performing assigned tasks, must be competent in all of the following areas: Recipient Rights/Licensing and mandated reporting requirements. First Aid. Cardiopulmonary Resuscitation/AED. Supervision and protection. Prevention and containment of communicable diseases. Safety and fire prevention. The Federal Deficit Reduction Act and Medicaid False Claims Act. Abuse and neglect.
Knowledge of youth development, Positive Youth Development principles, housing stability services, trauma-informed care, case management, resource coordination, and family-centered practice.
Must pass all necessary training required by the State, any governmental agency, or the employer. Successful completion of training regarding Human Trafficking, Suicide Awareness, and Trauma-Informed Care.

Responsibilities

Conduct assessments to identify strengths, barriers, housing stability needs, and service needs of youth and families.
Develop and implement individualized service plans utilizing a youth-centered and family-driven approach.
Provide ongoing case management services to support housing stability and prevent homelessness.
Maintain regular contact with youth and families as required by funding source and agency standards.
Provide information, referrals, advocacy, and linkage to community resources.
Assist youth and families in accessing housing, education, employment, healthcare, transportation, and other supportive services.
Support family engagement, mediation, and conflict resolution when appropriate.
Facilitate life skills development activities and independent living skill instruction.
Coordinate flexible financial assistance and supportive services in accordance with program policies and funding requirements.
Provide crisis intervention and safety planning as needed.
Participate in on-call responsibilities as assigned.
Assist youth and families in identifying and maintaining safe and stable housing.
Collaborate with landlords, housing providers, schools, employers, and community partners to support housing stabilization efforts.
Assist youth and families with applications for housing, benefits, and other supportive services.
Advocate on behalf of youth and families experiencing barriers to housing stability.
Monitor housing outcomes and service utilization.
Maintain ongoing liaison with community agencies, organizations, schools, landlords, and individuals relevant to program services.
Disseminate information to the community and special target groups regarding available services.
Participate in outreach activities designed to identify and engage youth and families in need of services.
Advocate for consumers with various community agencies.
Strengthen interagency relationships by participating in information sharing activities, joint training activities, and collaborative service delivery.
Attend community organization meetings, coalitions, and planning groups as assigned.
Participate in multidisciplinary teams and collaborative service planning efforts.
Document client contacts and case activity as required by funding sources and agency standards.
Maintain accurate, complete, and timely case records.
Complete all required assessments, service plans, progress notes, and discharge documentation.
Assist in monthly, quarterly, annual, and grant reporting activities.
Collect and report program outcome data as required by funding sources.
Conduct eligibility screenings, intake assessments, and ongoing assessments of consumer needs.
Develop service plans utilizing person-centered and family-centered planning principles.
Provide life skills training and supportive services as needed.
Maintain ongoing communication to provide consumers with needed services, resources, and opportunities.
Implement service plans designed to maximize the consumer's physical, social, cultural, educational, vocational, and emotional well-being.
Conduct follow-up assessments documenting consumer progress, changes, and outcomes.
Develop discharge and aftercare plans to support continued stability and success.
Promote youth voice, choice, and leadership opportunities.
Encourage participation in Positive Youth Development activities.
Assist with Youth Advisory Board recruitment, engagement, and facilitation activities as assigned.
Support youth involvement in community engagement and leadership opportunities.
Practice universal precautions, as exposure to blood-borne or airborne pathogens may occur due to direct contact with others.
Participate in staff meetings, training, and professional development activities.
Other duties as assigned.
