HeartShare St. Vincent’s Services (HSVS) Nuevo Camino program is dedicated to providing 38 unaccompanied minors, ages 5-12, with a supportive, nurturing environment through transitional foster homes, daily educational services Monday - Friday, and supportive wrap-around services in the Sunset Park neighborhood section of New York City. Our program is rooted in ensuring that service delivery is trauma informed, culturally appropriate, and tailored to the needs of each child while complying with ORR (Office of Refugee Resettlements) program requirements. Nuevo Camino is seeking a case manager to provide oversight and support to these young persons. The Case Manager's major responsibilities are to identify an appropriate sponsor, either parents or guardians, to whom the child may be released/unified with. The Case Manager assesses the safety and well-being of potential sponsors and identifies any concerns, discussing them with the Nuevo Camino team and Federal Field Specialist. The Case Manager is also responsible for ensuring that the child in the transitional home is receiving essential services such as: medical and dental care, mental health, and basic education, orientation, and legal screening.
