JASA is seeking a part-time Case Manager to work two days a week. The Case Manager will follow the standards of the social work profession and the requirements of the funding source. The Case Manager is responsible for assessing the social and emotional needs of older adult clients and their families, developing and implementing case plans, providing information about available services, screening clients for benefits eligibility, arranging direct services, evaluating clients' capacity to manage their affairs, referring clients to other community agencies, conducting ongoing reassessment of clients’ needs, informing supervisors of crises, conducting group counseling and community education groups, representing and interpreting the agency at community conferences and meetings, participating in training and unit meetings, and completing required case records, reports, and statistics.
