About The Position

Canyon Ridge Hospital is currently seeking a compassionate and talented Case Manager to join our team. At Canyon Ridge Hospital, we understand that not all wounds are visible. Depression, anxiety, trauma, PTSD, addiction, and suicide affect millions of individuals and families. Our mission is to be a leader in behavioral healthcare, dedicated to excellent service in the community. Canyon Ridge Hospital has serviced the Greater Inland Empire since 1989. Canyon Ridge is a 157-bed acute, locked psychiatric facility offering services to adolescents ages 13-17, adults, and older adults. We believe in an individualized, patient-focused approach to diagnosis and treatment. For some, anxiety or drug dependence looks different from others with the same diagnoses, and in some cases, behavioral health and substance use disorders occur simultaneously. In these cases of dual diagnosis, we treat the patients in the best way possible given their specific needs. This position is temporary for 3-5 months with a potential of going Full Time.

Requirements

  • Masters degree in Social Work
  • Written and oral communication skills
  • Knowledge of utilization management
  • Skills in case management
  • Time management skills
  • Discharge planning skills
  • Crisis intervention skills common to acute psychotic as well as to non-violent crises intervention practice
  • Knowledge of acute adult psychological disorders
  • Advanced principals of abnormal psychology as specifically applied to adults, geriatrics, adolescents

Nice To Haves

  • 2 years of experience in inpatient setting preferred

Responsibilities

  • Responsible for providing quality case management, utilization review activities and psychotherapeutic services to psychiatric patients and their families, spouse, significant other.
  • Serve as a member of the interdisciplinary team supporting the organization’s treatment program and philosophy.
  • Assure the deliverance of quality treatment to psychiatric patients and their families.
  • Assessing clients' needs and developing individualized care plans.
  • Coordinating with healthcare providers, social services, and other stakeholders.
  • Monitoring and evaluating clients' progress and adjusting care plans as needed.
