About The Position

Join an innovative mobile team focused on serving members of our community! The Mobile Health Team will serve individuals and families in the field, meeting them where they are at. These teams will operate out of a mobile van that will cover Clark County and include Mental Health Professionals, Case Managers, Medical Providers, and Peer Support. Columbia River Mental Health Services provides comprehensive behavioral health and recovery services that transform the lives of children, adults and families in the communities of SW Washington. We are the largest and oldest not-for-profit provider of mental/chemical dependency treatment in southwest Washington with 200 employees and a history dating back to 1942. Our Mission is: Providing behavioral health and recovery services that transform the lives of children, adults, and families in the communities of SW Washington. Case Managers work with consumers who have mental illness. They are part of a multi-service treatment team. Case Managers are often the point person for the treatment team and help facilitate and coordinate services within and without CRMHS. Case Managers offer education about mental illness, assist consumers with achieving their goals, and help with skill building to promote independence and recovery. Case Managers work with individuals, family members, significant members of social and support networks and lead a variety of psycho-educational groups. Case Managers may work with their clients in the clinic, in the home, and in the community.

Nice To Haves

  • Bachelor's degree in Mental Health, Social Work, Psychology, or Human Services
  • Approximately 1-2 years of experience in the social service field with mental illness or chemical dependency

Responsibilities

  • Work collaboratively with each client to develop a unique treatment plan based on the individual’s recovery goals, addressing life domains such as Mental Health, Alcohol/Drug Issues, Activities of Daily Living, Work/School, Family/Natural Support, Income/Finances, Social/Friends/Fun, Legal, Medical and Dental, Housing/Shelter, Cultural/Spiritual, and Other.
  • May teach and assist to improve living, coping, symptom management, communication and interpersonal skills.
  • Collaborate and coordinate care with other treatment providers and help connect with legal, court, and community resources.
  • Provide assistance with crises and help with development of crisis plans before the need arises.
  • Help clients connect to community health care providers and assist with medical and dental appointments.
  • Offer education about mental illness and treatment to clients, family members and other significant members of the client’s support system.
  • Provide support and encouragement in client’s ability to work, go to school, develop friendships and extend the social network.
  • Advocate for client’s rights and fair treatment in all areas of life.
  • Maintain all required paperwork/charting requirements as established by agency contracts.
  • Achieve and maintain agency standard productivity requirements.
  • Supports agency mission, goals, and objectives.
  • Attend staff and planning meetings as scheduled.
  • Maintain communication and cooperative working relationships both within the agency and community at large, while maintaining a high degree of confidentiality.
