In 1982, Options was established to respond to the need for housing in the community for people with serious mental illness. Today, Options serves over 2,000 adults and children annually and manages more than 160 residential properties across Long Island. Options for Community Living, Inc. is looking for a Residential Support Case Manager to join our Mobile Residential Support Team! The Residential Support Case Manager provides supportive case management services to tenants with mental illness in supportive housing and provides on-site visits to scattered site locations. The Case Manager will work with the tenant, treatment team, and the Mobile Residential Support Team (MRST) to identify issues and barriers that may prevent the individual from maintaining stability in the community. Through a mutually agreed goal plan, the MRST Case Manager will assist the tenant in seeking solutions to achieve mental health and health care goals, sobriety, housing stability and to prevent where possible, emergency room usage and inpatient hospitalization.
