About The Position

Options for Community Living, Inc. is committed to helping Long Island’s most vulnerable families and individuals live healthier, more stable, and productive lives. In 1982, Options was established to respond to the need for housing in the community for people with serious mental illness. Today, Options serves over 2,000 adults and children annually and manages more than 160 residential properties across Long Island. Options for Community Living, Inc. is looking for a Residential Support Case Manager to join our Mobile Residential Support Team! The Residential Support Case Manager provides supportive case management services to tenants with mental illness in supportive housing and provides on-site visits to scattered site locations. The Case Manager will work with the tenant, treatment team, and the Mobile Residential Support Team (MRST) to identify issues and barriers that may prevent the individual from maintaining stability in the community. Through a mutually agreed goal plan, the MRST Case Manager will assist the tenant in seeking solutions to achieve mental health and health care goals, sobriety, housing stability and to prevent where possible, emergency room usage and inpatient hospitalization.

Requirements

  • High School Diploma or the equivalent and two years of experience working with case management responsibilities in a setting with disabled or economically disadvantaged individuals and/or families.
  • Valid driver’s license, safe driving record, valid auto insurance and access to a vehicle is required.

Nice To Haves

  • Bachelor's degree in human services related discipline or two years of college in a human service related discipline is preferred.

Responsibilities

  • Provide site visits, write progress notes collaboratively with the tenant (when possible), review service plan and goals.
  • Help develop independent medication management skills.
  • Provide linkages to community resources.
  • Assist in securing vocational assessment services, job training and education programs.
  • Provide advocacy services to gain appropriate entitlements and services.
  • Intervene in situations requiring immediate attention to ensure safety of residents.
  • Provide mediation, counseling, behavioral intervention and crisis management services.
  • Consult and coordinate with clients’ family and providers as needed for coordinated service delivery.
  • Notify Program Supervisor of any medical or psychiatric emergency.
