About The Position

Gulf Coast Center (GCC) is seeking a PRN On-Call Case Manager for the Mobile Crisis Outreach Team (MCOT). This role provides rapid response mobile crisis services in the community for adults and children experiencing mental health and substance use crises. Responsibilities include crisis assessment and intervention, intensive crisis follow-up, rehabilitative skills training, family education, community resource referrals, and transportation. The position also assists individuals in the Crisis Respite Program and responds to calls from law enforcement, jails, or judges for jail diversion assistance. Teamwork, trauma-informed care, jail diversion, and collaborative documentation are essential. Effective communication with various agencies and the ability to work under pressure are required. Staff must adhere to GCC training requirements and state/federal regulations.

Requirements

  • Bachelor's degree from an accredited college or University in the Behavioral Science field of study.
  • General knowledge of mental health disorders.

Responsibilities

  • Provide rapid response mobile crisis services in the community.
  • Conduct crisis assessment and intervention for adults and children experiencing mental health and substance use crises.
  • Provide intensive crisis follow-up services until clients are stabilized and transitioned to regular services.
  • Offer rehabilitative skills training, family education, and community resource referrals.
  • Provide transportation to community resources for crisis stabilization and continuity of care.
  • Assist individuals in the Crisis Respite Program with assessment, recovery planning, and rehabilitative services.
  • Respond to calls from law enforcement, jails, or judges requesting assistance with mentally ill and/or IDD individuals for jail diversion.
  • Assist jail-diverted individuals with crisis assessment, LOC5 enrollment, clinic linkage, referrals, and follow-up services.
  • Respond to crises at Gulf Coast Center clinics and facilities as requested.
  • Use designated crisis assessment, safety planning tools, and progress notes.
  • Conduct Adult Needs and Strengths Assessment or Children's Need and Strengths Assessment.
  • Complete Crisis Assessment tool in the Smartcare electronic health record collaboratively with the client by the end of the shift.
  • Communicate with the full crisis services team by email about completed crisis assessments and needed follow-up services.
  • Enter Crisis Intervention service notes and Mental Health Screening and Triage service notes in the Smartcare EHR within 24 hours of service.
  • Enter documentation for crisis relapse and prevention follow-up services in the EHR by the end of shifts.
  • Serve as primary staff for disaster recovery deployment and maintain training for Psychological First Aid.
  • Participate in a rotation for disaster assistance at Disaster Recovery Centers, emergency shelters, and other sites.
  • Adhere to and promote agency compliance policy.
  • Communicate with various departments to foster and develop community partnerships.
  • Treat all individuals with courtesy, decency, and respect.
  • Provide responsive and efficient services.
  • Interact with staff, consumers, local entities, and advocacy groups.
  • Provide the highest level of customer service.
  • Report personnel leave, flex time, and emergency leave to assigned supervisor.
  • Adhere to professional, ethical, and legal requirements set forth in GCC's Business Code of Conduct and Corporate Compliance Plan.
  • Adhere to the Center's Dress Code.
  • Adhere to Center safety guidelines and expectations.
  • Serve on the after-hours crisis rotation as needed.
  • Accurately complete bi-weekly timecards, travel logs, vehicle reports, and assigned reports.
  • Ensure completion of initial and annual ANSA/CANS recertification.
  • Participate in semi-annual quality assurance training activity.
