About The Position

The Healthy Start Fatherhood Initiative Case Manager (HSFICM) is responsible for comprehensive case management within the Healthy Start Fatherhood Initiative (HSFI) parenting education program and related fatherhood initiatives. This role serves fathers with children aged 0–18 months, ensuring their unique needs are addressed during the critical early stages of parenting. The HSFICM will facilitate parenting education, outreach, and data compliance while also providing direct support in financial stability, employment readiness, and resource navigation. This position requires strong leadership, project management, and communication skills, as well as experience working with families in low-income communities.

Requirements

  • At least 1 year of combined direct service experience working with men and families.
  • Experience with clients facing substance abuse, mental health, employment, criminal justice, and other issues.
  • Experience with adult education and/or group facilitation.
  • Bachelor’s degree in social work, child development, psychology, or related field.
  • Must pass child abuse, criminal history, and FBI clearance.
  • Must be able to travel in Philadelphia and to be available to work some evenings and weekends as needed.
  • Ability to work independently and as part of a team.
  • Strong organizational skills and ability to multitask.
  • Ability to complete directives and meet deadlines required.
  • Experience providing direct services to men and families, preferably with infants or young children.
  • Experience with adult education and group facilitation.
  • Strong knowledge of issues related to child development, fatherhood, healthy relationships, public health, financial empowerment, and workforce development.
  • Excellent written and oral communication skills.
  • Proficient with MS Office, email, and web-based data systems.
  • Comfortable working with adults in low-income communities and those facing barriers related to employment, financial security, or health.

Responsibilities

  • Carry a caseload of participating fathers, focusing on those with children aged 0–18 months.
  • Conduct comprehensive intake, parenting, financial, and employment assessments to identify participant strengths, needs, and barriers.
  • Develop individualized service plans with participants, including parenting, financial, employment, and educational goals.
  • Facilitate parenting education workshops using the National Fatherhood Initiative’s 24/7 Dad curriculum, both in-person and virtually.
  • Maintain regular contact with participants (in-person, virtually, or by phone), providing ongoing support, referrals, and follow-up.
  • Identify and set goals with participants; refer to community resources, including trauma counseling, housing, employment, health, legal, and social services.
  • Maintain the highest level of confidentiality and data compliance, ensuring accurate record-keeping in the program’s ETO database.
  • Collaborate with staff, partner organizations, and community stakeholders to recruit participants and market the program.
  • Track participant progress and prepare required program reports and documentation.
  • Stay current with best practices in parenting education, maternal/child health, financial empowerment, racial disparities, infant development, mental health, trauma-informed care, and related fields.
  • Participate in regular supervision, staff meetings, conferences, and professional development opportunities.
  • Meet with participants in diverse settings, including homes, community sites, online, and group classes.
  • Contribute to program fidelity and compliance with organizational and funder standards.
  • Perform other duties as assigned.
