About The Position

Requirements

Valid Licensed Vocational Nurse (LVN) license in the state of California.
Minimum of 2 years of clinical nursing experience, preferably in a hospital or healthcare setting.
Basic knowledge of case management principles and patient care coordination.
Strong understanding of medical terminology and healthcare documentation standards.
Current CPR certification.
Excellent interpersonal and communication skills to effectively interact with diverse patient populations.
Ability to work collaboratively within multidisciplinary teams.
Clinical nursing skills daily to assess patient conditions and implement care plans effectively.
Strong communication skills are essential for educating patients and coordinating with healthcare professionals to ensure continuity of care.
Organizational skills enable the management of multiple cases simultaneously while maintaining accurate and timely documentation.
Problem-solving abilities are applied to address barriers to care and facilitate resource access for patients.
Proficiency with electronic health records supports efficient information management and compliance with healthcare regulations.

Responsibilities

Conduct thorough patient assessments to identify healthcare needs and develop individualized care plans.
