Case Manager - Low Intensity Care Coordinator

About The Position
This position is based at our Children's Outpatient Clinic in Dothan Monday-Friday 8am-5pm.
Requirements
- Must have a BS degree in Psychology or related mental health field
- Must have a good driving record free from DUI/reckless driving for the past (3) years- will be subject to initial and ongoing MVR checks
- Must successfully complete case management training through Alabama DMH and receive certification of that training upon hire
Responsibilities
- Interview and determine needs of adolescents/families within the Houston County area
- Assist consumers in accessing community services inside and outside of the SpectraCare System
- Implement service plans
- Service as a liaison between consumers of the program and other community agencies
- Maintain client records using an electronic clinical record system
