About The Position
Provides individualized support to program participants, provides individual support in the community, connecting them to healthcare, behavioral health, and community resources, ensuring continuity of care, and supports program operations.
Requirements
- Bachelor’s degree in social work, counseling, or related field (Master’s preferred).
- Experience working with pregnant women, mothers, and families.
- Knowledge of community resources and case management best practices.

Responsibilities
- Conduct comprehensive assessments and develop individualized care plans.
- Provide referrals to healthcare, behavioral health, housing, childcare, and other community services.
- Track participant progress, follow up regularly, and adjust plans as needed.
- Maintain accurate and timely documentation in compliance with organizational and funder requirements.
