The Justice-Involved (JI) Case Manager works with individuals who are transitioning from incarceration. Justice-involved individuals—those who are currently, or have previously been, in jails, youth correctional facilities, or prisons—face a higher risk of injury and death compared to the general population. The Case Manager will assist patients to understand recommendations given by their health care providers, perform patient care functions, answer, and screen medical calls; post patient information to medical records and provide support in planning continuity of treatment.
