About The Position

Enhanced Care Management (ECM) Case Manager Location: Redding & Eastern Shasta County Monday-Friday 8:30am to 5:00pm POSITION SUMMARY Under the supervision of the ECM Program Manager and/or designated leadership staff, the Enhanced Care Management (ECM) Case Manager delivers community-based care coordination and case management for individuals with complex medical, behavioral health, housing, and social service needs. This fast-paced role offers meaningful work that supports rural communities by helping clients access care, housing, behavioral health services, community resources, and hope. The ECM Case Manager works collaboratively within an integrated care setting to support clients in accessing services, improving health outcomes, increasing stability, and reducing barriers to care. This position serves clients throughout Redding and Eastern Shasta County and requires both field-based and office-based work, including electronic documentation and team based collaboration.

Requirements

  • High School Diploma or GED required.
  • Minimum of one (1) year experience in case management, social services, behavioral health, healthcare, community health, or related field.
  • Experience working with diverse and underserved populations.
  • Basic computer proficiency including Microsoft Office, email, and electronic health records documentation systems.
  • Ability to work independently and as part of a multidisciplinary team.
  • Valid California Driver’s License, reliable transportation, and proof of insurance required.

Nice To Haves

  • Associate’s Degree (AA) or Bachelor’s Degree (BA/BS) in Social Work, Psychology, Human Services, Public Health, Behavioral Health, or related field preferred.
  • Experience working in integrated healthcare, community health, or managed care settings preferred.
  • Knowledge of Enhanced Care Management (ECM), Community Supports, Medi-Cal systems, or care coordination programs preferred.

Responsibilities

  • Provide comprehensive case management and care coordination services to clients.
  • Conduct outreach, engagement, assessments, and ongoing follow-up with clients in community, clinic, home, and office settings.
  • Develop and maintain individualized care plans in collaboration with clients and interdisciplinary care teams.
  • Coordinate services with medical providers, behavioral health providers, community agencies, housing programs, and other support systems.
  • Assist clients with accessing healthcare, benefits, transportation, housing resources, and social services.
  • Maintain accurate, timely, and compliant electronic documentation, including case notes, assessments, referrals, and care plans.
  • Participate in interdisciplinary team meetings, case conferences, and program training.
  • Support clients using trauma-informed, culturally responsive, and client-centered approaches.
  • Monitor client progress and update care plans as needed.
  • Maintain confidentiality and comply with HIPAA and organizational policies and procedures.
  • Establish and maintain positive working relationships with community partners and team members.
  • Perform other duties as assigned in support of ECM program operations.
