The Case Manager III provides comprehensive services to homebound elderly clients in the community through support services to maintain the client at home with a safe care plan. This role involves conducting in-home assessments, developing and implementing care plans, providing crisis intervention, performing intake activities, and completing necessary documentation. The Benson Ridge and the Southwest Queens Senior Case Management Programs offer case management and homecare services to the homebound elderly. These services include care management plans, assessments for homecare, and connections with Home Delivered Meals and Friendly Visiting. The primary goal is to provide vital support services to enable frail seniors to live safely in their homes and communities.
