About The Position

The Case Manager III position at the St Paul Opportunity Center, within the Homeless Elders Program, focuses on providing in-person outreach and housing-focused case management to older adults experiencing homelessness. The primary goal is to assist these individuals in obtaining safe, stable housing and appropriate support services. This role involves person-centered care for vulnerable community members, managing a caseload of approximately 20 older adults in shelter or unstable housing. Responsibilities include working with clients to overcome housing barriers and achieve other goals outlined in their care plans. Additionally, the role may involve conducting trainings for internal and external providers on working with homeless older adults.

Requirements

  • Minimum of 1 year of experience working with older adults, people with disabilities and/or people experiencing ongoing impact of poverty.
  • If working in a Housing Support Program: Experience as a Minnesota Department of Human Services certified peer specialist.
  • Meet requirements of unlicensed personnel under Minnesota Statutes sections 144A.43 to 144A.483.

Nice To Haves

  • Bachelor’s Degree in Social Work, Psychology, Human Services or related field is preferred.
  • In lieu of degree, a minimum of five (5) years of experience working with a specific client group is required.

Responsibilities

  • Provide comprehensive case management services including intake, interview and assessment, goal setting, and information and referral services.
  • Provide assessment services and short-term counseling, skill building techniques, and aftercare.
  • Provide client services in a variety of outreach settings both at agency sites and in the community.
  • Responsible for documentation, reviewing and ensuring the relevance, accuracy, and timeliness of data in client files.
  • Make appropriate referrals and provide comprehensive decision-making counseling to help ensure client’s needs are being met.
  • Enroll clients in Housing Stabilization Services for billing purposes as appropriate.
