SUMMARY DESCRIPTION OF WORK The Heartland Homes program provides facility-based Permanent Supportive Housing in the form of long-term financial assistance and supportive services using a Housing First model to individuals and families experiencing long-term homelessness and those who have a disability. This program has two facilities located in Council Bluffs, IA that this position would work out of. Heartland Homes targets families with high service needs, providing intensive case management services. The Case Manager will work to assess, verify eligibility, and provide long-term case management/ supportive services to clients in the program. The Case Manager also works closely with the Property Manager.
