About The Position

With Safe Haven I, the Case Manager II helps individuals build stability, develop life skills, and take meaningful steps toward greater independence. Through personalized support, resource coordination, and psychosocial rehabilitation services, you'll empower clients to overcome challenges, achieve their goals, and create a stronger foundation for the future. The Case Manager II provides in-unit training and support for new Case Managers. The CM II also helps to develop training curricula, processes, and resources and ensures that training is evidence-based and complies with regulatory, accreditation, and organizational requirements.

Requirements

  • High School diploma or equivalent
  • Nine months of certified case management experience
  • Case Manager certification
  • CPR certification
  • Valley de-escalation certification
  • Driving positions require a minimum age of 21 and a current driver’s license

Responsibilities

  • Completes training checklists and reports progress to unit leadership, and serves as a Valley Academy classroom instructor as assigned
  • Coaches, assesses progress, and provides individualized support as necessary to address opportunities for improvement.
  • Provides case management services to assigned caseload and meets client care expectations
  • Functions as a member of the interdisciplinary treatment team by recommending specific interventions to support existing treatment goals and objectives when appropriate.
  • Distributes medications as needed after required training
  • Follows agency documentation policies for all phases of treatment, including assessments, care plans, treatment transitions and discharge plans, and client care.
  • Collaborates with internal and external multidisciplinary teams regarding clinical aspects of treatment.
  • Adheres to ethical and legal standards set by licensing boards and state regulations.
  • Meets direct care expectations.
  • Participates in ongoing professional development activities to meet licensure, certification, and/or other agency requirements
