This position serves as a Float Case Manager and is assigned to the program site(s) with the greatest operational need. Assignments may vary and may change based on staffing, program demands, and client needs. This position provides direct case management to adults with serious mental illness and co-occurring conditions who are experiencing homelessness or housing instability. The Float Case Manager II works alongside clients to assess needs, develop individualized service plans, access resources, and strengthen skills for long-term stability. They serve as both advocate and connector - helping clients navigate complex systems of care while promoting dignity, safety, and independence. The role collaborates closely with internal teams and community partners to ensure clients receive consistent, trauma-informed support.
