About The Position

People Incorporated is seeking a Case Manager II to join their Waiver Case Management Anoka County team. This role involves working with clients diagnosed with chronic mental illness, supporting them in connecting with resources to live independently, and developing self-awareness for positive future planning through frequent community meetings. Case Managers operate autonomously, creating their own schedules, and focus more on behind-the-scenes support for client needs. The organization values work-life balance with manageable caseloads averaging around 30 clients. Requirements

  • Valid driver’s license, excellent driving record and current auto insurance
  • Bachelor’s Degree from an accredited 4-year college with a major in social work, psychology, sociology or a closely related field; OR
  • Bachelor’s Degree from an accredited four-year college with a major in any field, AND One year of experience as a social worker/case manager/care coordinator in a public or private social service agency

Nice To Haves

  • Bachelor’s Degree in Social Work, Special Education, Psychology, Nursing, Human Services or other fields related to the education or treatment of persons with developmental disabilities or related conditions, AND One year of experience in waiver case management or care coordination with adults experience severe mental illness.

Responsibilities

  • Perform functional assessments that lead to individual plan development and result in the client receiving access to necessary mental health and related services.
  • Conduct regular review of plan effectiveness and make adjustments as needed that result in the client receiving the maximum amount of assistance and support while monitoring the effectiveness of services received.
  • Assessing client needs and ensuring continuity of care.
  • Making ongoing referrals to resources based on assessment and current client need.
  • Maintaining necessary daily records for continuity of service and reports to funding agencies.
  • Excels at documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards.
  • Attending client monthly meetings to monitor services.
