The ECHO Case Manager II works directly with pregnant and parenting teenagers to support their educational success and personal growth. This position helps participants achieve their high school education goals by providing case management, advocacy, life skills education, resource coordination, and ongoing support. The Case Manager partners with youth to navigate challenges, connect with community resources, develop parenting and independent living skills, and create pathways toward long-term self-sufficiency and success. Summary of work: Responsible for conducting assessments, writing case plans and providing case management services according to department needs, guidelines, and procedures. Meet with clients in schools, hospitals, home visits, and/or other locations as deemed necessary. Refers families in need of supportive services, and completes appropriate paperwork.
