About The Position

The Case Manager II will conduct comprehensive case management services to persons receiving outpatient mental health services under the DMH funded program within Jwch Institute Inc. The CM II will participate as a member of a multidisciplinary field-based team to complete intake and assessment, targeted case management, care coordination, rehabilitation services, crisis stabilization and referrals to community-based programs for persons experiencing mental illness homelessness, and co-occurring condition.

Requirements

  • AA or bachelor’s degree in Social Work, sociology or related field experience with at least 1-year experience as care coordinator, community health worker or similar responsibilities
  • High School Diploma or GED with at least 3 years’ experience as care coordinator, housing navigation, community health worker or similar responsibilities
  • Willing and able to work in low-income areas of Los Angeles with persons experiencing mental illness substance use, and are homeless or at risk of homelessness.
  • experience working in the Skid Row community or similar high-need, unhoused, low income, underserved environments
  • Knowledge of trauma-informed care, harm reduction, and Housing First principles
  • Strong organizational, time management and documentation skills
  • Cultural humility and ability to work with diverse populations
  • Experience with Electronic Health Records (EHR) systems
  • Experience working in a DMH funded program
  • Valid California driver’s license

Responsibilities

  • Conducts comprehensive case management services, including intake and assessment, development of a comprehensive care plan, implementation of the care plan, specialty referrals, and follow-up.
  • Connects clients to support services and provides appropriate referrals including but not limited to medical, addiction medicine and recovery programs, dental, transportation and housing.
  • Collaborate with multidisciplinary team to address the needs of persons living with mental illness, substance abuse disorders, complex medical conditions, or experiencing or at risk of homelessness. Communicate client progress, needs, and risks to the team
  • Advocates on behalf of clients with healthcare professionals, utilizing motivational interviewing, trauma-informed care, and harm-reduction approaches
  • Coordinate a range of medical and psychosocial services to address social, emotional, and practical needs
  • Support with crisis intervention and de-escalation in community settings
  • Coordinate outreach activities in and outside of the clinic
  • Conduct field based work to assist clients with skills building when navigating environmental difficulties.
  • Assist clients with housing stabilization and retention
  • Maintain productivity requirements to meet contract compliance.
  • Complete all charting as required by funding sources, including assessments, progress notes, care plans, and billing documentation on timely basis
  • Collect necessary data for reporting purposes, as requested
  • Participate in agency and community meetings
  • Adhere to all organizational safety policies, procedures, and regulatory requirements to ensure a safe environment for patients, staff, and visitors.
  • Participate in required safety and compliance trainings and apply learned practices in daily work activities.
  • Identify, report, and help mitigate unsafe conditions, incidents, or hazards in the workplace.
  • Use equipment, tools, and supplies safely and responsibly in accordance with organizational and regulatory standards.
  • Maintain awareness of infection control, occupational health, and emergency preparedness procedures appropriate to the role.
  • Participate in JWCH’s customer service training called AIDET on an annual basis.
  • Demonstrate an understanding of AIDET (Acknowledge, Introduce, Duration, Explanation, and Thank You) to apply in patient encounters or team discussions.
  • Utilize the AIDET communication framework to communicate with patients and team members in a manner that reduces patient anxiety, increases patient compliance, and improves clinical outcomes.
  • Perform other duties as assigned.
