About The Position

The Case Manager is responsible for assessing, coordinating, and connecting individuals and families to services that support their individualized needs. This role focuses on advocacy, resource navigation, and short-term support to ensure access to behavioral health, social services, and community-based resources.

Requirements

  • Bachelor’s degree in Human Services or related field required
  • Intermediate computer skills required
  • Valid driver’s license (minimum 3 years, non-provisional) required
  • Access to a reliable personal vehicle with state minimum insurance
  • Child abuse clearances and other state-required background checks required
  • Strong communication and interpersonal skills
  • Ability to collaborate with diverse populations and community partners
  • Ethical practice and commitment to confidentiality
  • Strong organizational and documentation skills
  • Problem-solving and crisis intervention abilities
  • Knowledge of behavioral health systems and community resources
  • Cultural humility and person-centered approach
  • Proficiency in basic computer systems and documentation tools

Nice To Haves

  • 1 year of case management experience preferred
  • 2 years of experience supporting individuals with behavioral health, substance use, or IDD preferred

Responsibilities

  • Collaborate as part of a multidisciplinary team to deliver holistic, person-centered services
  • Build and maintain relationships with community providers and partner agencies
  • Provide referrals to behavioral health and community-based services, including RHD programs and external resources
  • Assess individual needs and determine service eligibility through office and community-based interactions
  • Provide screenings, referrals, and resource coordination in a timely manner
  • Assist with benefits and services including Medicaid, Social Security, housing, employment, and transportation
  • Offer short-term, solution-focused support to promote independence and community integration
  • Complete all required documentation in compliance with RHD, funders, and regulatory standards
  • Perform responsibilities in accordance with County Administrative Case Management (ACM) requirements
  • Maintain accurate records and ensure confidentiality (HIPAA compliance)
  • Attend required trainings, meetings, and maintain professional standards
