About The Position

The Case Manager II will act as a member of a Community Public Health Team (CPHT) to provide case management services, follow-ups, referrals, advocacy, and wraparound services to clients. The mission is to transform trauma and violence into empowerment, advancing systemic healing, and developing leaders for lasting change.

Requirements

  • Must be a highly organized individual able to multi-task, track and document case management of clients.
  • Computer competency, including Word, Excel, Outlook and PowerPoint.
  • Must demonstrate the ability to use communication skills that successfully convey and obtain accurate and complete information in a respectful and caring manner.
  • Have a basic knowledge of body language and non-verbal communication.
  • Understand and demonstrate active listening skills.
  • Must be a critical thinker, reasoning, organizing and analyzing information so that problems are accurately understood and solutions are congruent with client’s capacities and outcome orientated.
  • Capacity to negotiate and collaboration to insure the client’s successful receipt of services.
  • Demonstrate the skill of advocating for the needs and rights of people.
  • Must show a passion and sensitivity for trauma work.
  • Demonstrate experience to maintain cooperative working relationships with others; be team oriented; coordinate/collaborate with professionals from multiple agencies whilst maintaining appropriate boundaries and client confidentiality.
  • Access to a reliable vehicle, valid California Driver’s license and proof of automobile insurance

Nice To Haves

  • Has experience working with adults, families and/or children in the community or experience in community organizing or in a social service setting is preferred.
  • Bilingual English/Spanish preferred.

Responsibilities

  • Develop in partnership with community members, and multi-disciplinary team partners, client centered, strength-based case management plan.
  • Meet with program clients referred through the household needs assessment to assist in removing barriers to meet case management goals and linking clients to on and offsite resources.
  • Liaise: Work in partnership and coordination with CPHT partner agency, Northeast Valley Health Corporation (NEVHC) and designated Case Managers.
  • Develop comprehensive needs assessments, coordination of services, and provides education.
  • Develop goals based upon client stated needs and identification of problem(s), will evaluate and monitor progress.
  • Provide systems navigation, linkages to referrals for health care, mental health services or other social services, provide collateral resources, and monitor client progress.
  • Assist Clinicians in coordinating additional resources for clients as needed.
  • Ensure client’s basic needs are met, outside of mental and physical health, providing a continuum of care and wrap around services.
  • Provide Assertive Outreach.
  • Collect and analyze health data.
  • Maintain accurate documentation, monthly reports, inputting data into the data management systems.
  • Complete required reports for funders, responsible for meeting grant milestones.
  • Participate in weekly multidisciplinary meetings to build capacity and develop sustainability plans.
  • Attend staff meetings, participates in training of staff, volunteers, and the community.
  • Performs other duties as assigned.
