About The Position

The Case Manager II will act as a member of a Community Public Health Team (CPHT) to provide case management services, follow-ups, referrals, advocacy, and wraparound services to clients. The position reports to the Program Manager - Prevention and Outreach Services. The case manager II will develop, in partnership with community members and multi-disciplinary team partners, a client-centered, strength-based case management plan. The case manager II will meet with program clients referred through the household needs assessment to assist in removing barriers to meet case management goals and linking clients to on and offsite resources. This role involves working in partnership and coordination with CPHT partner agency, Northeast Valley Health Corporation (NEVHC) and designated Case Managers. The role includes developing comprehensive needs assessments, coordination of services, and providing education. Goals will be developed based upon client stated needs and identification of problem(s), and progress will be evaluated and monitored. Client services include systems navigation, linkages to referrals for health care, mental health services or other social services, providing collateral resources, and monitoring client progress. The role assists Clinicians in coordinating additional resources for clients as needed and ensures clients' basic needs are met, outside of mental and physical health, providing a continuum of care and wrap-around services. Assertive outreach is also a component of this role. Data collection and documentation are crucial, including collecting and analyzing health data, maintaining accurate documentation, monthly reports, and inputting data into data management systems. The role is responsible for completing required reports for funders and meeting grant milestones. The Case Manager II will participate in weekly multidisciplinary meetings to build capacity and develop sustainability plans, attend staff meetings, and participate in training of staff, volunteers, and the community.

Requirements

  • Experience working with adults, families and/or children in the community or experience in community organizing or in a social service setting is preferred.
  • Highly organized individual able to multi-task, track and document case management of clients.
  • Computer competency, including Word, Excel, Outlook and PowerPoint.
  • Demonstrate the ability to use communication skills that successfully convey and obtain accurate and complete information in a respectful and caring manner.
  • Basic knowledge of body language and non-verbal communication.
  • Understand and demonstrate active listening skills.
  • Critical thinker, reasoning, organizing and analyzing information so that problems are accurately understood and solutions are congruent with client’s capacities and outcome orientated.
  • Capacity to negotiate and collaboration to insure the client’s successful receipt of services.
  • Demonstrate the skill of advocating for the needs and rights of people.
  • Show a passion and sensitivity for trauma work.
  • Demonstrate experience to maintain cooperative working relationships with others; be team oriented; coordinate/collaborate with professionals from multiple agencies whilst maintaining appropriate boundaries and client confidentiality.
  • Must be a thorough department background investigation and live scan.

Responsibilities
