About The Position

People Incorporated is seeking a Case Manager II - Waiver Case Management to work with clients diagnosed with chronic mental illness. The role supports clients in connecting with resources to live as independently as possible and helps them develop self-awareness for positive future planning through frequent community meetings. Case Managers operate autonomously, creating their own schedules. This is a Full-Time position, Monday-Friday, with business hours from 8:00 AM to 4:30 PM. The role is located in Anoka County, Minnesota, and offers a hybrid work environment. A hiring bonus of $1,000 is available to external candidates. Requirements

  • Valid driver’s license, excellent driving record and current auto insurance
  • Bachelor’s Degree in Social Work, Special Education, Psychology, Nursing, Human Services or other fields related to the education or treatment of persons with developmental disabilities or related conditions
  • One year (2,000 hours) of experience in the education or treatment of persons with developmental disabilities or related conditions

Responsibilities

  • Perform functional assessments that lead to individual plan development and result in the client receiving access to necessary mental health and related services.
  • Conduct regular review of plan effectiveness and make adjustments as needed that result in the client receiving the maximum amount of assistance and support while monitoring the effectiveness of services received.
  • Assessing client needs and ensuring continuity of care.
  • Making ongoing referrals to resources based on assessment and current client need.
  • Maintaining necessary daily records for continuity of service and reports to funding agencies.
  • Documenting all interventions, interactions, and relevant information to the treatment plan consistent with professional and organizational standards.
  • Attending client monthly meetings to monitor services.
