About The Position

People Incorporated is seeking a Case Manager II to join our Waiver Case Management (Hennepin County) team. This role involves working with clients diagnosed with chronic mental illness, supporting them in connecting with resources to live as independently as possible. The position helps clients develop self-awareness for future planning through frequent community meetings. Case Managers build long-term relationships with clients and operate autonomously, creating their own schedules. This is a full-time position with flexible business hours, Monday-Friday, 8 am-4 pm. The role requires an onsite presence within the Twin Cities, Minnesota, but offers hybrid with remote opportunities. Requirements

  • Valid driver’s license, excellent driving record and current auto insurance
  • Bachelor’s Degree in Social Work, Special Education, Psychology, Nursing, Human Services or other fields related to the education or treatment of persons with developmental disabilities or related conditions
  • One year of experience (2,000 hours) in the education or treatment of persons with developmental disabilities or related conditions

Responsibilities

  • Perform functional assessments that lead to individual plan development and result in the client receiving access to necessary mental health and related services.
  • Conduct regular review of plan effectiveness and make adjustments as needed that result in the client receiving the maximum amount of assistance and support while monitoring the effectiveness of services received.
  • Assessing client needs and ensuring continuity of care.
  • Making ongoing referrals to resources based on assessment and current client need.
  • Maintaining necessary daily records for continuity of service and reports to funding agencies.
  • Documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards.
  • Attending client monthly meetings to monitor services.
