About The Position

People Incorporated is seeking a Case Manager II to join our Targeted Case Management Dakota County team to primarily work with clients who are diagnosed with chronic mental illness or other chronic health conditions. We connect people with services through developing person-centered support plans that help them live as independently as possible. Our Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules. This is a Full-Time position, Monday - Friday, during business hours, with a hybrid work arrangement in Dakota County, Minnesota. The salary range is $24.25 - $25.75 Hourly, based on qualifications. External candidates may be eligible for a $1,000 hiring bonus. Requirements

  • Passing all background and requirements for facility and position
  • The ability to pass the screening required by Federal Human Services Office of Inspector General to assure that person has not been excluded from working in the Health Care Programs which receive federal funding.
  • Must have a valid and current driver’s license and auto insurance.
  • Valid driver’s license, excellent driving record and current auto insurance
  • Bachelor’s Degree in behavioral science or related field
  • 2,000 hours of supervised experience working with adults with severe and persistent mental illness
  • OR Master’s Degree in behavioral science or related field.

Nice To Haves

  • Experience providing direct services to adults with serious and persistent mental illness
  • Experience with case management and commitment proceedings
  • Knowledge of Recovery, Motivational Interviewing and Person-Centered approach

Responsibilities

  • Must be able to have onsite presence within Dakota County, Minnesota
  • Attending bi-annual meetings with clients to monitor services
  • Develop and maintain a collaborative relationship with clients serving as a role model for clients by demonstrating positive, proactive and respectful communications
  • Assessing client needs and ensuring continuity of care. This may include outreach and consultation with other partners and providers.
  • Maintain regular contact with collaborative partners and make referrals to professionals, schools and community agencies, as needed
  • Making referrals and authorizing services based on needs
  • Maintaining records and documenting all coordination activities
  • Excels at documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards
