About The Position

People Incorporated is seeking a Case Manager II to join our Targeted Case Management Metro Counties team. This role primarily works with clients diagnosed with chronic mental illness or other chronic health conditions. The Case Manager II connects people with services through developing person-centered support plans that help them live as independently as possible. Our Case Managers develop long-term relationships with their clients and operate autonomously, creating their own schedules. This is a full-time position, Monday - Friday, during business hours, serving the Twin Cities Metro Counties in Minnesota. The role offers a hybrid work arrangement.

Requirements

  • Valid driver’s license, excellent driving record and current auto insurance
  • Bachelor’s Degree in Social Work, Special Education, Psychology, Nursing, Human Services or other fields related to the education or treatment of persons with developmental disabilities or related conditions
  • One year of experience in the education or treatment of persons with developmental disabilities or related conditions

Responsibilities

  • Must be able to have onsite presence within the Twin Cities, Minnesota
  • Attending bi-annual meetings with clients to monitor services
  • Develop and maintain a collaborative relationship with clients serving as a role model for clients by demonstrating positive, proactive and respectful communications
  • Assessing client needs and ensuring continuity of care. This may include outreach and consultation with other partners and providers.
  • Maintain regular contact with collaborative partners and make referrals to professionals, schools and community agencies, as needed
  • Making referrals and authorizing services based on needs
  • Maintaining records and documenting all coordination activities
  • Excels at documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards
