The Hotspotting Opioid Program is an evidence-based program in which a care management model will be led by certified recovery peer advocates and clinical professionals with the objective of providing person-centered services to those at risk of opioid overdose in a safe and collaborative way. The program’s focus is to reduce opioid harm, promote a continuum of recovery, and address critical social determinant of health factors. Under the supervision of the Director of Harm Reduction, the individual will be responsible for providing client outreach, interventions for clients, provide crisis consultations, maintain client case records, linkages to care, referral follow up. Case Managers will document interventions according to all funder and agency requirements, submit statistical data and reports, attend weekly supervision, participate and facilitate case conferences, conduct staff trainings on substance abuse topics, and perform other additional tasks as delegated by supervisor.
