Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. Telecare In-Home Outreach Team (IHOT) is a centralized program with three mobile teams providing in-home outreach. IHOT serves to 240-300 adults per year with serious mental illness (SMI).
